Prologue
Travis Grady is on his way to Brahms, but his chat on his CB is cut short as his signal mysteriously breaks up mid-conversation. It is raining heavily and visibility is not great, especially at night. However, as weary as Travis may be, he notices a hooded figure staggering out into the middle of the road.
Strange visions pass through his mind, and as he fights to gain a mental focus, he must also fight to gain control of his massive truck that is speeding toward what appears to be a helpless pedestrian. Travis jams on his brakes. His trailer begins to skid, pushing his rig on the edge of jackknifing.
Luckily, or perhaps not, he is able to stop in time to prevent his truck from crushing the stranger in the street. After he exits the vehicle, he looks around for the wayward person. Nobody is lying in the street, but when he glances into the side view mirror of his cab, he sees a ghostly image of a girl surrounded by fog. He turns to confront the child, but she is not there.
Travis sees the girl again and gives chase, following her to a burning house.
Plot
Silent Hill: Origins starts with the crashing of Travis Grady's truck in response to the sudden appearance of a hooded figure in the road ahead of his truck. Travis follows the image of young girl, who leads him to a burning house. Inside the house, he finds the girl, who has third degree burns over one hundred percent of her body. He carries her body from the house and then collapses in the front yard.
When Travis awakens, he is lying on a bench in the Fog World. He goes to Alchemilla Hospital in search of the burned girl, where he meets a suspicious doctor who claims the child is not there. The girl appears in one of the bathroom mirrors, causing the world to shift from the Fog World to the Otherworld. Travis navigates back and forth between these worlds, until he leaves the hospital.
Travis then travels to Cedar Grove Sanitarium, where he meets a woman named Dahlia Gillespie. Travis begins to find memos suggesting that his mother, Helen Grady, used to stay at the sanitarium, after she attempted to kill both herself and Travis. At the end of the level, Travis faces the Memory of Helen Grady, from which he obtains a piece of the Flauros.
Travis is led to the Artaud Theater, where he fights Ariels. He is led to the Caliban, a monstrous representation of Alessa Gillespie's fear of Shakespeare's The Tempest.
Travis then goes to the Riverside Hotel. There, he discovers more repressed memories from his past, including a memory of his father committing suicide. He fights Straightjackets and Two Backs as he goes back and forth between the various hotel rooms. In the final room, he confronts the Memory of Richard Grady, which is a twisted version of his dead father.
He returns to Alchemilla Hospital and assembles the Flauros, which releases Alessa's powers, allowing her to cause the Otherworld to overtake Silent Hill. Travis uses a map drawn by the seven-year-old Alessa to find the sanctuary of the cult. He sees the various cult members standing over Alessa's burned and blackened body and attempts to fight them, but he is incapacitated by Dr. Kaufmann. Travis is then sent into the Flauros, where he faces the Demon, a powerful entity held captive within the Flauros.
After defeating the Demon, Travis leaves town. As he looks in his rearview mirror, he sees Alessa holding the infant Cheryl Mason.
Various voices are then heard, including that of Dahlia lamenting the splitting of Alessa's soul and devising a plan to call the split half of the soul back to the town and Harry Mason commenting on finding a baby on the side of the road.
